  • Report your valid U.S. passport lost or stolen immediately. Protect yourself from identity theft.
  • Use one of three ways to report it lost or stolen:
    • Online 
    • By mail, or 
    • In person when you apply for a new passport. 
  • After you report your valid passport lost or stolen, you cannot use it for international travel even if you find it later. 
  • Do not report a passport lost or stolen if it has expired.

Want to replace your valid lost or stolen passport? Reporting it lost or stolen does not replace it. You must apply in person if you want a replacement passport. 

Ways to Report your Lost or Stolen Passport

Visit our Online Form Filler to report your passport lost or stolen. When reporting online, we cancel your passport within one business day. We will send you a confirmation email after it is canceled.

Report a Passport Lost or Stolen, Submit DS-64 Online

 

Fill out online, print, sign, and mail Form DS-64 to the address on the form. You will also need to submit a photocopy of the front and back of your photo ID (example: driver’s license) if you are not applying for a new passport. It may take several weeks to receive your report and cancel your passport. 

When you report your valid passport lost or stolen while applying for a new passport at an acceptance facility, we may not cancel your passport for several weeks after you submit the application. To immediately cancel your passport, visit our Online Form Filler.

You will need to complete Form DS-11 if you want to report a passport lost or stolen at the same time you apply for a new one:

  • On the form, you will need to provide detailed information about where and when your most recent passport was lost or stolen.
  • Provide a copy of the police report if you filed one at the time the passport was lost or stolen.
  • Please include all relevant details in the space provided on Form DS-11. If you need more space to provide details, include another sheet of paper.
  • If you do not provide all this information on Form DS-11, we may pause your application and ask you to submit Form DS-64.

If you do not report your new passport was lost in the mail within 120 days after we issued it, you must apply again and pay all fees.

No. If your passport expired, it is no longer valid and you cannot use it for international travel. You should not report an expired passport as lost or stolen.

No. No one can travel using only a U.S. passport number. You must present an original, physical version of a passport to travel.

The passport book and card have many security features to prevent successful counterfeiting. 

You should only report your valid U.S. passport lost or stolen if the physical document is lost or stolen. Once you report a U.S. passport lost or stolen, it is no longer valid and you cannot use it for international travel.

You can return the passport of a deceased relative for cancelation. After we cancel it, we will return the passport if requested. 

Please submit:

  • The valid passport
  • A certified copy of the death certificate 
  • A letter requesting the cancelation and return (or destruction) of the passport

No. After you report your passport lost or stolen, we cancel it and it is no longer valid for travel. If you try to use the passport you reported as lost or stolen, you may be delayed while traveling and denied entry to a foreign country.
