Report Your Passport Lost or Stolen

Report your valid U.S. passport lost or stolen immediately to protect yourself from identity theft. Do not report an expired passport as lost or stolen.

Ways to report your lost or stolen passport

We will cancel your passport after you report it lost or stolen. It will no longer be valid for travel even if you find it later.

Online

The fastest way is to visit our Online Form Filler to report your passport lost or stolen. We will cancel your passport within 1 business day and send you a confirmation email. 

By mail

Fill out Form DS-64 online. Then print, sign, and mail it to the address on the form. 

You must submit a photocopy of the front and back of a photo ID, like your driver’s license.

This option takes longer. It may take us several weeks to receive your report and cancel your passport. To immediately cancel your passport, visit our Online Form Filler.

While applying for a new one

You can report your valid passport lost or stolen while applying for a new one using Form DS-11.

It may take several weeks for us to cancel your passport after you submit your new passport application. To immediately cancel your passport, use our Online Form Filler. You must include information about your most recent passport while filling out Form DS-11:

  • Share the details of where and when you lost it or had it stolen
  • If you filed a police report, provide a copy.
  • If you need more space to provide details, include another sheet of paper.

If you do not provide all relevant information on Form DS-11, we may pause your application and ask you to submit Form DS-64.  

Special situations

No one can travel using only a U.S. passport number. You must present an original, physical version of a passport to travel.

The passport book and card have many security features to prevent successful counterfeiting. 

You should only report your valid U.S. passport lost or stolen if you lose the physical document or someone steals it. Once you report a U.S. passport lost or stolen, it is no longer valid. No one can use it for international travel.

Use the Online Passport Status System to confirm if we mailed your passport and check your tracking number.

If it’s been more than 2 weeks since we mailed your passport

We will help you complete Form DS-86 which is a special form that confirms you didn't receive your U.S. passport. You must complete this form within 120 days of the date we issued your passport. After 120 days, you must reapply and pay all fees, by law.

If you lost your valid U.S. passport in a natural disaster, you may be able to replace it for free.

If you find another person's lost U.S. passport, mail it in a sturdy envelope to:

You can return the passport of a deceased relative for cancelation. After we cancel it, we will return the passport if requested. 

Submit:

  • The valid passport
  • A certified copy of the death certificate
  • A letter requesting the cancelation and return or destruction of the passport
